From January 1st, 2020, to March 1st, 2021, a retrospective analysis of 88 VVLE patients treated at the Third Hospital of Shandong Province was undertaken. Patients undergoing varied treatments were separated into corresponding study and control groups. The 44 patients in the study cohort experienced the concurrent procedures of ultrasound-guided foam sclerotherapy and endoluminal radiofrequency closure. The control group, consisting of 44 patients, had high ligation and stripping of the great saphenous vein. The postoperative venous clinical severity score (VCSS) of the affected extremity and the postoperative visual analog scale (VAS) score were considered efficacy indicators. We assessed the influence of South Africa's Centralized Chronic Medication Dispensing and Distribution (CCMDD) program, part of its differentiated ART delivery approach, on clinical outcomes by comparing viral load suppression and retention rates in patients enrolled in the program to those managed through the clinic's standard care protocol.
Patients living with HIV, whose clinical state was stable and who met the criteria for differentiated care, were enrolled in the national CCMDD program and tracked for a period of up to six months. In a secondary analysis of trial cohort data, we examined the relationship between routine patient participation in the CCMDD program and their clinical outcomes of viral suppression (<200 copies/mL) and continued care involvement.
From a population of 390 people living with HIV (PLHIV), 236 (61%) were evaluated for Chronic and Multi-Morbidity Disease Diagnosis and Disease Management (CCMDD) eligibility. Following evaluation, 144 (37%) were determined eligible, and, ultimately, 116 (30%) of those found eligible enrolled in the CCMDD program. Of the CCMDD visits (286 total), 265 (93%) resulted in timely ART acquisition for participants. There was a negligible difference in VL suppression and retention in care between CCMDD-eligible patients who participated in the program and those who did not (adjusted relative risk [aRR] 1.03; 95% confidence interval [CI] 0.94–1.12). A comparison of CCMDD-eligible PLHIV program participants and non-participants revealed no significant difference in VL suppression (aRR 102; 95% CI 097-108) and retention in care (aRR 103; 95% CI 095-112).
The CCMDD program effectively provided individualized care to clinically stable participants. PLHIV who participated in the CCMDD program maintained a high level of viral suppression and continued care, showcasing the effectiveness of the community-based ART delivery model in ensuring positive HIV care outcomes.
By employing differentiated care strategies, the CCMDD program successfully assisted clinically stable participants. Viral suppression and continued engagement in care remained high among individuals with HIV participating in the CCMDD program, implying the community-based model of ART provision did not have a detrimental effect on their HIV care outcomes.
